29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) Maintenance Shop: On or about October 25, 2022, and at times prior, the employer exposed employees to struck-by hazards, in that the employer did not ensure each operator had successfully completed training consisting of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace before operating the "Toyota" model 8FGCU25 forklift (Serial Number 39410).

29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Maintenance Shop, Wash Bay Area:  On or about October 25, 2022, and at times prior, the employer exposed employees to struck-by hazards, in that the employer did not ensure the employees used effective eye protection while exposed to flying particles during pressure washing inside trailers.
