29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 21, 2022, employees were exposed to a fall of approximately 16 feet while performing roofing activities on a residential home located at 708 10th Street NE, Massillon, Ohio 44646. The employees were working on all areas of the roof and the employer did not ensure they were using fall protection. Pecha Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1499297, citation number 1, item number 2, and was affirmed as a final order on (11/13/20), with respect to a workplace located at 3414 Amherst Ave. NW, Massillon, Ohio 44648.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about October 21, 2022, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 708 10th Street NE, Massillon, Ohio 44646. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area. Pecha Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1499297, citation number 1, item number 3, and was affirmed as a final order on (11/13/20), with respect to a workplace located at 3414 Amherst Ave. NW, Massillon, Ohio 44648.
