29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about October 25, 2022, employees were exposed to a struck by hazard while performing framing activities on a commercial building located at 10305 Cascade Crossing, Brooklyn, Ohio 44144. Employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

Employees were working from the top plate of the 2nd level and the employer did not ensure they were using fall protection.
