(a) 404 South Boulder Hwy, Henderson, Nevada: On or about May 20, 2022, an employee sustained a work-related injury where an act of harassment and intimidation caused the employee to experience unconscious. On or about May 25, 2022, the work-related injury resulted in work restrictions from a licensed health care professional. The work-related injury was not recorded on the OSHA Form 300 or equivalent, Job transfer or restriction, column (I).  (b)	404 South Boulder Hwy, Henderson, Nevada: On or about September 28, 2022, an employee sustained a work-related injury where a USPS vehicle accident caused the employee to receive prescription medication from a licensed health care professional. The work-related injury was not recorded on the OSHA Form 300 or equivalent, Other recordable cases, column (J).  The US Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.(4)(a), which was contained in OSHA inspection number 1331929, citation number 1, item number 1 and was affirmed as a final order on 09-20-2018, with respect to a workplace located at 1611 Spring Gate Lane, Las Vegas, Nevada.  The US Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.(4)(a), which was contained in OSHA inspection number 1248704, citation number 1, item number 1 and was affirmed as a final order on 10-23-2018, with respect to a workplace located at 3375 South Rainbow Blvd., Las Vegas, Nevada.
