On or about October 18, 2022, at the establishment located at  905 South State Street, Merrill, Wisconsin; at Dock 38, the employer did not ensure that measures, such as wheel chocks or sand shoes, were used to prevent a semi-trailer on which a dock leveler was placed from moving during unloading operations, thereby exposing a worker to fall hazards that resulted in a hospitalization.
