29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment. a) Worksite,1105 Walnut Street, Wilkinsburg PA, On or about October 18, 2022- Employees were exposed to 48-feet fall hazards while removing tile from a residential multi pitch roof. The employees were not trained on the proper use and operation of the personal fall arrest equipment, in that employees failed to attached their harnesses to lanyards and the roof chimney was used as an anchor point.
