29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) Heavy Plate Department - Employees operating the BR-9 metal forming roll were exposed to amputation hazards in that ingoing nip points were not guarded, on or about October 27, 2022.  (b) Heavy Plate Department - Employees operating the BR-7 metal forming roll were exposed to amputation hazards in that ingoing nip points were not guarded, on or about October 27, 2022.  (c) Heavy Plate Department - Employees operating the GS-19 metal shear were exposed to amputation hazards in that the point of operation was adequately guarded, on or about October 27, 2022.  (d) MP Department - Employees operating the GS-10 metal shear were exposed to amputation hazards in that the point of operation was adequately guarded, on or about October 27, 2022.  (e) Shipping Department - Employees operating a miter saw were exposed to amputation hazards in that the guard had been taped open, on or about October 27, 2022.    Abatement Certification and Documentation Required.

(b) Machine Shop - Employees operating the DoAll and Kyser Johnson horizontal bandsaws were exposed to amputation hazards in that the section of blades not in use were not guarded, on or about October 27, 2022.
