29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a.  On the roof at a residence, located at 1571 South Shore Drive, Fleming Island, Florida:   On or about 10/26/2022, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating saws and drills to secure roof shingles.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.  On the masonry property wall at a residence, located at 1571 South Shore Drive, Fleming Island, Florida:   On or about 10/26/2022, the employer exposed employees to fall hazards ranging from 6 to 9 feet, in that employees were not protected from falling from the masonry wall as they stepped from the wall in order to access the interior ladder inside the property as they accessed the roof.

On the roof at a residence, located at 1571 South Shore Drive, Fleming Island, Florida:   On or about 10/26/2022, the employer exposed employees to fall hazards ranging from 6 to 30 feet, in that the side rails of a ladder were not extended at least 3 feet above the roof while employees were ingressing and egressing the roof.  b.   On the masonry property wall at a residence, located at 1571 South Shore Drive, Fleming Island, Florida:   On or about 10/26/2022, the employer exposed employees to fall hazards ranging from 6 to 9 feet, in that the side rails of a ladder were not extended at least 3 feet above the wall while employees were ingressing and egressing the wall as they stepped from the ladder.
