29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) RotaTherm - An employee performed maintenance work on a pneumatic butterfly valve and did not place a lockout device on the air cut-off valve, on or about October 27, 2022. b) RotaTerm - An employee performed maintenance work on a pneumatic butterfly valve and did not place a lockout device on the electrical isolator panel, on or about October 27, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) RotaTherm - The employer's energy control procedure did not contain a specific statement on the intended use of the procedure, on or about October 27, 2022. b) RotaTerm - The employer's energy control procedure did not clearly and specifically outline the procedural steps for shutting down, isolating, blocking and securing the machines hydraulic, mechanical, and stored mechanical hazardous energy sources, on or about October 27, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
