29 CFR  1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:  a) The employer did not report an amputation that was the result of a work-related incident that occurred on or about July 22, 2022, to OSHA within twenty-four (24) hours.  The injury was reported to OSHA on October 25, 2022.  No abatement certification or documentation is required for this item.

29 CFR  1910.132(h)(5): The employer did not provide replacement Personal Protective Equipment to employees when such equipment was required for protection.  a) On or about October 27, 2022, the employer did not provide replacement personal protective equipment, such as hearing protection, gloves and safety glasses, at no cost to the employees.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
