29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about October 27, 2022, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 1709 Sapphire Drive, Hudson, Ohio 44236. Employees were working on upper areas of the roof above employees working on lower levels with tools and materials that could have fallen, and the employer did not ensure that lower employees were wearing protective helmets.

Employees were working all areas of the roof and the employer did not ensure that they were trained on how to recognize and minimize fall hazards.

Employees were working all areas of the roof and the employer did not ensure that they were using fall protection. Wolff Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1500996, citation number 1, item number 1, and was affirmed as a final order on 12/10/20, with respect to a workplace located at 311 Sand Run, Akron, Ohio 44313.
