29 CFR  1910.217(b)(4)(ii): Mechanical power press(es) were not provided with a pad with a nonslip contact area firmly attached to the pedal:  a.  CQT Kennedy, LLC, worksite located at 1260 Industrial Drive, Van Wert, Ohio 45891: on or about October 27, 2022, the employer did not ensure the  foot pedal on the Chicago part revolution mechanical power press, Model 810 L, S/N  L18558,  was equipped with nonslip pad to prevent unintended cycling during production, there by exposing employees to amputation hazards.

29 CFR  1910.217(b)(7)(iii): A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) was not supplied with the clutch/brake control to select type of operation of the press. Fixing of selection was not by means capable of supervision by the employer.  a.  CQT Kennedy, LLC, worksite located at 1260 Industrial Drive, Van Wert, Ohio 45891: on or about October 27, 2022, the employer did not ensure Chicago part revolution mechanical power press, Model 810 L, S/N  L18558,  cycle selection switch keys were under the control of a supervisor. During set up and production, an operating employee with unabated access to inch mode selection was exposed to amputation hazards.

Employees operating the machine without the benefit of instruction and oversight of point of operation protection(s) were exposed to amputation hazards.

29 CFR  1910.217(c)(1): The employer did not ensure the usage of "point of operation guards" or properly applied and adjusted point of operation devices were performed:  CQT Kennedy, LLC, worksite located at 1260 Industrial Drive, Van Wert, Ohio 45891: On or about October 21, 2022, the employer did not ensure employees were protected from entering the point of operation while stamping parts on the Chicago part revolution mechanical power press, Model 810 L, S/N  L18558. Without these protections in place, employees performing these tasks were exposed to amputation hazards.

CQT Kennedy, LLC, worksite located at 1260 Industrial Drive, Van Wert, Ohio 45891: On or about October 27, 2022, the employer did not establish and follow as required, an inspection program for the Chicago part revolution mechanical power press, Model 810 L, S/N  L18558. Employees operating the press were not benefited by a systematic program of periodic and regular inspection of the press to ensure that all their parts, auxiliary equipment, and safeguarding were in safe operating condition and adjustment prior to use.
