29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about July 25, 2022, the employer did not develop energy control procedures when employees were required to perform servicing and maintenance on machines such as, but not limited to, the Aquaflex BXP 1306(4) machine. Employee(s) were exposed to hazardous energy which resulted in an employee injury. There were no alternative measures provided such as machine guarding. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about July 25, 2022, and after the employer did not train authorized employee(s) (operators) who were assigned and performed work on the Aquaflex BXP 1306(4) machine. Employee(s) who perform servicing and maintenance did not have the knowledge of the hazards of energy sources and were exposed to amputation hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
