On or about July 25, 2022, the employer did not develop energy control procedures when employees were required to perform servicing and maintenance on machines such as,  but not limited to, the Aquaflex BXP 1306(4) machine. Employee(s) were exposed to hazardous energy which resulted in an employee injury. There were no alternative measures provided such as machine guarding.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employee(s) who perform servicing and maintenance did not have the knowledge of the hazards of energy sources and were exposed to amputation hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
