On or about 10/26,2022, Isaias Roofing, dBa Isaias General Contracting, LLC was installing roofing materials on a residential building located at 1189 Riesling Lane in Pevely, MO. Four employees were observed on a 10/12 sloped roof, approximately 11-20 feet above the ground without utilizing fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface. On or about 10/26,2022, Isaias Roofing, dBa Isaias General Contracting, LLC was installing roofing materials on a residential building located at 1189 Riesling Lane in Pevely, MO. The extension ladder the employees were using to access the roof did not extend three feet above the landing surface or edge of the roof.
