29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: At the jobsite located at 17300 S Woodstone Dr, Olathe, KS 66062, employees engaged in residential roofing activities were exposed to fall hazards of approximately 11 feet from the roof peak to the ground below while accessing the roof. The portable extension ladder did not extend at least 3 feet above the upper landing or was not secured to the structure.

KCH Remodeling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1541630, citation number 1, item number 4, and was affirmed as a final order on 10/07/2021, with respect to a workplace located at 9000 Greely St, Lenexa, KS.
