OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that an employee was exposed to crushing and struck-by hazards while moving concrete formwork.  (a) On or about October 28, 2022, CD Smith Construction, Inc., 555 Kilbourne Ave, Milwaukee, WI:  An employee worked in the fall shadow of a stripped concrete wall form lifted by a GEHL Telehandler forklift (Model #rs8-42 gen:3) while attempting to maneuver the form to be laid down on the ground. The 1,890 pound wall form was suspended from a single hook at the end of the Telehandler forklift attachment using a Symons wall system lifting bracket attached to a steel choker.  An employee  pushed the bottom of the wall from the vertical lifted position while the form was elevated.  This condition exposed the employee to a crushing hazard.
