On or about October 29, 2022, in room MP15, the employer did not ensure all guards on the Tetra Pak Ice Cream Choice Filler A1, Serial Number Z5201420, were adequately secured to the machine.  The method of securing non-interlocked guarding along the bottom of the machine allowed the guards to be easy defeated as the hex style bolts used were untightened and capable of being removed by hand.  Guarding secured in this manner allowed access to the danger zone and exposed affected employees to struck-by and crushing hazards.

Affected employees were, thereby exposed to hazards associated with moving and rotating parts.    Assemblers, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR  1910.147(c)(6)(i), which was contained in OSHA inspection number 1546137, citation number 1, item number 1a and was affirmed as a final order on January 24, 2022, with respect to a workplace located at 2850 W. Columbus Drive in Chicago, IL 60652.
