(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY, AND WHEN THE LAST DAILY INSPECTION WAS CONDUCTED) a) At the site located 600m North of the intersection at Highlands Blvd and Jimmy Deloach Parkway: on or about October 28, 2022, and at times prior to, the employer exposed employees to struck by hazards and engulfment hazards in that daily inspections were not conducted by a competent person to protect employees from cave-ins or structural collapses for employees conducting pipe work in an excavation.
