29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: a. On the roof at a commercial building, located at 4455 Atlantic Boulevard in Jacksonville, Florida: On or about September 28, 2022, the employer exposed employees to an 11-foot fall hazard, in that fall protection was not utilized by the employees while removing leaves and debris from the roof and gutters. The Episcopal School of Jacksonville, Inc. was previously cited serious for a violation of this occupational safety and health standard 29 CFR 1910.28(b)(1)(i), which was contained in OSHA inspection number 1471101, citation number 1, item number 1 and was affirmed as a final order on September 4, 2020, which respect to a workplace located at 4455 Atlantic Boulevard, Jacksonville, Florida.

On the roof at a commercial building, located at 4455 Atlantic Boulevard in Jacksonville, Florida: On or about September 28, 2022, the employer exposed employees to a 11-foot fall hazard, in that the employees were not trained by the employer to recognize and avoid unsafe fall hazard conditions. The Episcopal School of Jacksonville, Inc. was previously cited other-than-serious for a violation of this occupational safety and health standard 29 CFR 1910.30(a)(3)(i), which was contained in OSHA inspection number 1471101, citation number 2, item number 1 and was affirmed as a final order on September 4, 2020, which respect to a workplace located at 4455 Atlantic Boulevard, Jacksonville, Florida.
