On or about October 17, 2022, at the facility located at 3200 East Pershing Street, Appleton, Wisconsin; an employee was engaged in servicing and maintenance activities on the Kloster Corporation Mixer (Type: 1-200#/MIN Mixer, Serial No.: MS-7846) and had not been trained in the recognition of hazardous energy sources, the type and magnitude of energy, and methods and means to isolate the hazardous energy. b. On or about October 19, 2022, at the facility located at 3200 East Pershing Street, Appleton, Wisconsin; an employee was engaged in servicing and maintenance activities on the Kloster Corporation Mixer (Type: 1-200#/MIN Mixer, Serial No.: MS-7846) and had not been trained in the recognition of hazardous energy sources, the type and magnitude of energy, and methods and means to isolate the hazardous energy.
