29 CFR 1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. On or about October 27, 2023 at the 330 Highland Avenue Construction Project, an employee was working from an extension ladder and was leaning past the side rails of the ladder and fell off the ladder. The employee had not been trained on the use and limitations of the ladder.
