29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) South Side of House: On or about November 01, 2022, an employee who was not wearing head protection was exposed to head injuries from impacts by falling objects when he was passing four-foot by eight-foot sheets of roof decking up to a second employee on a roof.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) West Side of House: On or about November 01, 2022, the employer did not ensure that an employee used eye protection while he was exposed to flying particles while using a circular saw to rip sheathing.  b) West Side of House: On or about November 01, 2022, the employer did not ensure that an employee used eye protection while using a pneumatic nail gun to fasten sheathing to the side of the home being built.
