29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative: On or about November 11, 2022, throughout the facility, employees were performing maintenance and servicing work on machines and equipment without having an energy control program that included energy control procedures, employee training and periodic inspections, on equipment such as, but not limited to: a) TruBend brand 5230 press brake b) TruBend brand 5085 press brake c) Unknown brand roll bend d) Roundo brand pipe bender

29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: On or about November 1, 2022, and at times prior thereto, in the employee break room where a window unit air conditioner was installed into a wall and exhaust air vented into the employees break room causing condensation and mold. Proinlosa Energy Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.141(a)(3)(i), which was contained in OSHA inspection number 1588408, citation number 2 item number 3 and was affirmed as a final order on August 19, 2022, with respect to a workplace located at 633 Northpark Central Dr. Houston, TX 77073.
