29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Schindler Elevator Corporation, at a worksite located at 11013 Airport Hwy, Swanton, Ohio 43558: On or about October 31, 2022, the employer did not ensure energy control applications steps to control the electrical and mechanical energy sources on a Montgomery Escalator were implemented prior to an employee performing step removal activities. Employees performing these work task in an energized condition were exposed to caught in hazards while removing step bolts and removal of steps.

29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof: a. Schindler Elevator Corporation, at a worksite located at 11013 Airport Hwy, Swanton, Ohio 43558: On or about October 31, 2022, the employer did not ensure that all employees were safely positioned or removed from the pit area on the downside of a Montgomery Escalator, when the escalator was energized/jogged in order to re-position escalator steps for removal. Employees performing this activitiy in this condition were exposed to caught in hazards between the step and deck plate.
