(a) Toyota fork truck #F1, 6FDU40, had a horn that was disconnected and did not function when depressed and a bent and distorted backrest.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: 513 Betty Spring Rd, Gardner, MA: On or about November 1, 2022, employees utilized damaged extension cords that were not removed from service. One extension cord in the door shop was plugged into a wall outlet and was attached at the other end to a portable sander. The insulation at the plug end connected to the sander had become separated from the plug and had exposed conductors. An additional extension cord was plugged into a Weiders Red-D-Arc ZR-8 welder and had multiple cuts in the insulation throughout the length of cord exposing bare conductor.
