29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking / working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a. At the job-site located at 4962 Distribution Dr., Tampa, Florida: On or about November 1, 2022, employees were exposed to fall hazards of 25-feet while working on the roof of an industrial structure with steel roof deck, performing insulation and thermoplastic polyolefin (TPO) removal and installation. The roof had a 1:12 pitch; 25-feet to the eave. There were no guardrails, safety net, positioning devices, fall restraint systems or personal fall arrest system.

At the job-site located at 4962 Distribution Dr., Tampa, Florida: On or about November 1, 2022, employees were exposed to fall hazards of 25-feet while working on the roof of an industrial structure with steel roof deck, performing insulation and thermoplastic polyolefin (TPO) removal and installation. Employees were not instructed in the recognition and identification of fall hazards in the work area, the procedures to be followed to prevent falls to lower levels and the fall protection requirements for steel erection.
