The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) Location - 554 Turnpike Avenue, North Andover, MA On or about 11/01/2022, employees were utilizing a climbing type harness with a rope grab positioned at the front waist of the wearer, exposing the employees to injuries in the event of a fall.
