29 CFR  1926.304(d):When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position.  On or about November 1, 2022, employees operating a DeWalt skill saw with lower guard wedged in the up position while cutting framing material exposing them to cuts and scrape hazards.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about November 1, 2022, employees engaged in framing work without fall protection were exposed to unprotected fall hazards of approximately 10-feet to the ground below.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used.  On or about November 1, 2022, employees performing framing work utilized extension cords with the outer sheathing split exposing internal wiring. The employees were exposed to electrical shock hazards.  a) On the west side of the construction site, extension cord with outer sheathing split used to energize a DeWalt skill saw,  b)  On the east side, inside the building, extension cord with outer sheathing split used to energize a DeWalt skill saw.
