29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the worksite located at 300 Walnut Street, Kansas City, MO 64105: An employee engaging in painting was exposed to a fall hazard of approximately 12 feet 4 inches when a ladder used to ascend and descend from a ledge did not extend 3 feet above the upper landing surface.

29 CFR  1926.1053(b)(8):Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways were not secured to prevent accidental displacement, or barricaded to keep the activities or traffic away from the ladder:  At the worksite located at 300 Walnut Street, Kansas City, MO: An employee was exposed to a fall hazard when the extension ladder used was placed on a public sidewalk and not barricaded to keep outside traffic and activities away.
