29 CFR 1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) 2376 Main St. #812: On or about November 1, 2022 and at times prior, employees were exposed to cave-in hazards while performing trenching operations in an unprotected vertical wall excavation with previously disturbed soil that measured approximately 8 feet 8 inches in depth, 11 feet long, and 9 feet 7 inches wide. The employer failed to conduct a daily inspection of trench prior to allowing employees to enter.
