29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about October 27, 2022, and at times prior thereto, at the Floating Loopers of Line #5 Saturator, employees performed paper splicing and procedures were not developed, documented, and utilized for control of potentially hazardous energy that included but was not limited to gravitational (potential) energy from counterweight. Employees were exposed to unexpected release of hazardous energy.
