29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use. On or about November 2, 2022, the employer WS Trading LLC failed to maintain any safety data sheets for employees that change out the Liquified Petroleum Gas (LPG) or Propane on the Yale Veracitor 60VX forklifts used inside the facility.

29 CFR 1904.4(a): The employer failed to keep records of fatalities, injuries, and illnesses and must record each fatality, injury and illness that is work-related, a new case, and meets the general recording criteria. On or about November 2, 2022, the employer WS Trading LLC did not record any work related injuries or illnesses on their OSHA 300 log or an equivalent injury and illness log after the employer learned that an employee was injured and hospitalized as a result of a work-related incident.
