(a) Jinxed South Philadelphia located at 1915 Washington Avenue, Philadelphia, PA 19146: The employer did not ensure that the ABC (Dry Chem) fire extinguisher mounted on the wall near the cash register was subjected to an annual maintenance check. The extinguisher tag indicated it was inspected in October of 2017. Note: Applies to any extinguishers provided in the workplace. Maintenance check includes an internal examination, except extinguishers with stored pressure. Condition observed on/or about November 1, 2022.
