29 CFR 1910.1450(e)(3)(i):The employer's Chemical Hygiene Plan did not include the standard operating procedures relevant to safety and health considerations to be followed when laboratory work involved the use of hazardous chemicals. On or about October 8, 2022, employees were exposed to health hazards while discarding corrosive solutions. A chemical spill occurred as a result of the incorrect disposal of expired and concentrated (18M) Sulfuric Acid. An exothermic reaction resulted, spilling acid onto the ceiling and floor. The employer's Chemical Hygiene Plan did not include the standard operating procedures relevant to safety and health considerations to be followed when laboratory work involved the use of hazardous chemicals. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1450(f)(4)(i)(C): Employee training did not include the measures employees could take to protect themselves from the physical and health hazards of chemicals in the work area, including specific procedures implemented to protect employees from hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used: On or about October 8, 2022, employees were exposed to health hazards while discarding corrosive solutions. A chemical spill occurred as a result of the incorrect disposal of expired and concentrated 18M Sulfuric Acid (90-98%). An exothermic reaction resulted, spilling acid to the ceiling, lab hood, and onto the floor. The employer did not provide training that includes the measures employees could take to protect themselves from the physical and health hazards of chemicals in the work area, including specific procedures implemented to protect employees from hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
