OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. a) Site located at 2000 Potato Roll Ln. Valdosta, Georgia 31601: On or about October 25, 2022, and at times prior to, employees routinely place a 3-in threaded bolt by hand in the front hydraulic vice of a Vermeer D24x40III Directional Drilling Machine. The bolt is placed between the vice jaws and the lower drill rod to create more traction when the rod slips during the drilling operation. This action is conducted while the machine is on and operational, thereby exposing employees to caught-in and crush-by hazards. Among other methods, one feasible and acceptable means of abatement would be to follow the manufacturers lockout procedure, located on page 30-18 of the operators manual which states. "Lock out machine before entering an exit pit, changing tools, repairing drill rod, manually adding or removing drill rod, or performing other work on the drill string or tools."
