29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee was using appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a)  On November 1, 2022  employees who were required to use pneumatic nail guns to install roofing shingles, were not equipped with and required to wear eye protection.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  On November 1, 2022, at the above addressed jobsite, employees performing roofing work were exposed to the hazard of free falling from as high as (9) feet and contacting lower ground while using personal fall arrest systems that were not rigged to prevent these hazards.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee:  a)  On November 1, 2022,  anchorages being used for attachment of personal fall arrest equipment were not installed so that they would be capable of supporting at least 5,000 pounds per employee attached.  RAA Interior & Exterior Inc. was previously cited for a violation of this occupational safety and health standard or an equivalent standard 29 CFR 1926.502(d)(15), which was contained in OSHA inspection number 1541451, citation number 1, item number 1b and was affirmed as a final order on September 16, 2021, with respect to a workplace located at 35-37 East Orchard Lane  Palatine, Illinois 60067.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
