Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about October 10, 2022 through November 3, 2022, at 1850 Osborn Avenue in Oshkosh, Wisconsin, employees were exposed to a fall hazard while working from a roof surface 30 feet above surface level without using fall protection equipment.  Employees were removing existing HVAC roof top units (RTUs) and installing upgraded RTUs.

On or about November 3, 2022, at 1850 Osborn Avenue in Oshkosh, Wisconsin, the employer did not provide fall protection safety training certification for an employee required to used fall protection safety equipment while working at heights greater than six feet on a roof surface.
