29CFR1910.22(b): The employer did not ensure that each walking-working surface can support the maximum intended load for that surface: (a) USS ET Plant, Flooded Elbow: On or about October 31, 2022, the employer did not check to see the weight carrying capacity of the intact plate steel roof (that was being removed) nor of the refractory material located just below the sheet steel roof of the flooded elbow. An employee fell through the refractory material after the roof sheet metal had been removed from a section of the roof.

29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking/working surface 4 feet (1.2 m) or more above a lower level is protected from falling by one of the systems listed in 1910.28(b)(15)(i)-(iii). (a) USS ET Plant, Roof of the Flooded Elbow: On or about 10/31/2022, employees were working to remove the metal roof of the flooded elbow and they were not protected from falling to the bottom of the flooded elbow by a personal fall arrest system. The fall hazard was about 16 feet.
