29 CFR  1910.213(c)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.  A)     In the facility maintenance shop, there was a Ridgid brand table saw that was not guarded by a hood over the saw blade.  Employees working with the saw were exposed to the portion of the blade that was above the material that they would cut.

29 CFR  1910.213(c)(2): Each hand-fed circular ripsaw was not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator.   A)     In the facility maintenance shop, there was a Ridgid brand table saw that did not have a guard that included a spreader to make sure any ripped material would not squeeze the saw blade and be thrown back at the operator.

29 CFR  1910.213(c)(3): Each hand-fed circular ripsaw was not provided with non-kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut.  A)     In the facility maintenance shop, there was a Ridgid brand table saw that did not have non-kickback fingers installed to help prevent employees being struck by material that the blade could throw back at them.

29 CFR  1910.263(e)(1)(viii): Every mixer was not equipped with a full enclosure over the bowl which is closed at all times while the agitator is in motion. Only minor openings in this enclosure, such as ingredient doors, flour inlets, etc., each representing less than 1 1/2 square feet in area, shall be capable of being opened while the mixer is in operation.  A)     In the main kitchen there was a counter mounted industrial Hobart mixer that did not have any kind of enclosure over the bowl to prevent employees from being struck by or caught in the agitator.

29 CFR  1904.40(a): Basic requirement. When an authorized government representative asked for the records required to be kept under Part 1904, copies of the records were not provided within four (4) business hours.  A)     During the opening conference the employer was asked to provide copies of their OSHA 300 log and OSHA 300A summary sheets for the years  2019, 2020, 2021, as well as a copy of the OSHA 300 log for 2022 to date.  The employer did not provide any of the requested documents.

29 CFR  1910.157(c)(1): The employer provided portable fire extinguishers and did not mount, locate and identify them so that they were readily accessible to employees without subjecting the employees to possible injury.  A)   Inside main electrical room 6, there was a portable fire extinguisher that was not properly mounted to a secure location and was stored on top of a counter.  Damage to the fire extinguisher and injury to employees could result from it being knocked over and falling from the counter.
