29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  18 Steuben Street, Meriden CT: On or about November 4, 2022, the employer did not ensure the workers on a residential roof more than 6 feet above any lower level were protected by the use of a fall arrest system.  Distinct sections of the roof were measured to be approximately 11.5 to 26.5 feet above the ground level.  Arizona Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1420385, citation number 1, item number 1a, and was affirmed as a final order on February 19, 2020, with respect to a workplace located at 51 North Street, Wolcott, CT 06716.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  18 Steuben Street, Meriden, CT:  On or about November 4, 2022, employees had not been effectively trained in procedures to be followed to minimize the risk of falling from a roof while removing and replacing shingles in that several employees on the roof were observed to be wearing personal fall arrest harnesses but they were not connected to ropes or lanyards attached to anchor points.  Distinct sections of the roof were measured to be approximately 11.5 to 26.5 feet above the ground level.  Arizona Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1420385, citation number 1, item number 1b, and was affirmed as a final order on February 19, 2020, with respect to a workplace located at 51 North Street, Wolcott, CT 06716.
