Broadway, Saint Louis, MO., employees were exposed to a struck by hazard. An employee was observed on the shop floor using a compressed air gun rated at 90 P.S.I. while cleaning without a chip guard in place.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: On or about 11/04/2022 at Bachman Machine Company in Saint Louis, MO., employees were exposed to an electrical hazard. The CSHO observed electrical panels and disconnect switches being blocked by barrels, buckets, and a storage table.

The hazard communication program has not been updated since 01/01/2000. In addition, the hazard communication program was not maintained at the workplace where employees worked with hazardous chemicals including Dykem Transparent.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked. On or about 11/04/2022 at a machine company located at 4321 North Broadway, Saint Louis, MO., employees were exposed to a chemical hazard. The employer failed to properly mark Dykem Transparent Stain, which was being stored and used from plastic unmarked spray bottles located in multiple work areas throughout the shop floor. The safety data sheet identifies the following hazards: flammable liquids category 2, skin corrosion / irritation category 2, serious eye damage category 1, carcinogenicity category 2 and specific target organ toxicity, single exposure category 3. In addition, the SDS states it should be stored locked up in its original container.
