(a) On or about 11/2/2022, 104 Contant Rd Charlotte Amalie,  VI  00802, a wooden portable (6 Ft Ladder) was used by employees (2) to gain access to the roof, and it did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, and the ladder was not secured at its top to a rigid structure, exposing employees to fall from a ladder.
