29 CFR  1926.451(g)(1):29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold were not protected by a personal fall arrest system;  a) 156 midland Ave, Saddle Brook NJ:  A subcontractor employee performing construction activities was exposed to a 16 foot fall while working from a ladder jack scaffold without the use of personal fall arrest system, or other means of fall protection, on or about 11/7/22.

29 CFR  1926.1053(b)(8):Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways were not secured to prevent accidental displacement, or barricaded to keep the activities or traffic away from the ladder:  a)  156 Midland Ave, Saddle Brook NJ Exterior: Employees placed a ladder jack scaffold  where ladders were not secured and/or barricaded to prevent accidental displacement. Ladders were place on the street, on or about 11/7/22

29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Building interior-roof framing: An employees was exposed to head injuries while installing windows from a ladder jack scaffold. The employee was not protected by a hard hat or protective helmet, on or about 11/7/22.   The Helpper Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1632929, citation number 1, item number 1 and was affirmed as a final order on 9-9-22, with respect to a workplace located at 250 lakeview Ave Paterson NJ 07503.

The Helpper Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1632929, citation number 1, item number 2 and was affirmed as a final order on 9-9-22, with respect to a workplace located at 250 lakeview Ave Paterson NJ 07503.
