Clark Testing, 1801 Route 51 South, Jefferson Hills, PA 15205: On or about November 1, 2022, the employer did not maintain a written personal protective equipment PPE Assessment of the job task. An employee sustained an injury (burns) when he was preparing to do torch cutting of a steel plate.
