FLX LINE-X, 875 State Route 5 And 20, Geneva, NY 14456.  On or about November 3, 2022, the employer did not develop and implement a written respiratory protection program when requiring employees to wear a 3M 7503 half-face air purifying respirator.  b. FLX LINE-X, 875 State Route 5 And 20, Geneva, NY 14456.  On or about November 3, 2022, the employer did not develop and implement a written respiratory protection program when requiring employees to wear an Allegro 9901 full-face supplied air respirator connected to an Allegro 9815-EF breathing air blower.   Abatement certification must be submitted for this item.
