(a) Boiler room - On or about November 5, 2022, the employer allowed unqualified employees to work on energized 480 volts circuit breaker, exposing employees to burn hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: (a) Boiler room - On or about November 5, 2023, the employer did not provide employees with PPE such as but not limited to safety glasses, face shields, and insulating gloves when working in and around the 480 volts electrical panel exposing them to an arc flash hazard.
