29 CFR 1910.28(b)(13)(i):The employer did not ensure that each employee was protected from falling by a guardrail system, safety net system, travel restraint system or personal fall arrest system when work was being performed less than 6 feet (1.6m) from the roof's edge. On or about November 5, 2022, employees were exposed to fall hazards in excess of 4' while working on cooling units on the roof at 9960 McCombs St., El Paso, TX 79924.

29 CFR 1904.39(a)(2):The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On or about November 5, 2022, an employee was admitted as an in-patient at University Medical Center of El Paso, 4815 Alameda Ave, El Paso, TX 79905 as a result of an at-work injury. The employer did not report the hospitalization to OSHA within 24 hours as required.
