1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  459 Granby St. South Hadley, MA 01075: On or about November 8, 2022, four employees were exposed to fall hazards between 6 and 10 feet above a lower level while performing roofing activities on a low-slope roof.  There was a safety warning line system in place but there was no employee acting solely as the safety monitor.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. 459 Granby St. South Hadley, MA 01075: On or about November 8, 2022, the employer had not prepared a written certification record of fall protection training record which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for his four employees exposed to falls while engaged in roofing the structure.
