(a) IF PacMac Department - On or about November 8, 2022, the established procedures for the application of energy control did not outline the steps to ensure the dissipation of heat has taken place to ensure employees are not exposed to burn hazards during the removal of heat bars on Pacman IF machines.

29 CFR 1910.147(c)(7)(iii)(A):Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures. (a)IF Pacmac Department - On November 3, 2022, the employer failed to ensure authorized employees were provided refresher training when assigned to work on new equipment that was sitting idle when they were hired exposing them to the unexpected energization or the release of stored energy during service maintenance and during the removal of heat bars.

1910.147(d)(6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a) (a )IF Pacmac Department - On or about November 8, 2022, employees were exposed to machine hazards associated with moving jaws and hot parts heated to 325 degrees when performing service and maintenance such as but not limited to the removal of heart bars of the IF Pacmac #2, attachment of screws to the thin plate on the inside of Pacmac #2 and during the attempt to manually place the bottom zipper back into position. The employer failed to implement energy control application steps such as LOTO devices were not applied to the electrical disconnect and the dissipation pneumatic air was not performed [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(5)(2)], and verification of isolations [(d)(6)], were not implemented to protect employees from machine servicing hazards.
