29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:     a) On November 9, 2022, at the above addressed jobsite, employees were exposed to eye injury while operating a pneumatic nail gun without eye protection during carpentry work.  b) On November 9, 2022, at the above addressed jobsite, employees were exposed to eye injury while operating a table saw without eye protection during carpentry work.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On November 9, 2022, at the above addressed jobsite, employees performing sheeting of a residential roof were exposed to fall hazards of approximately 20 feet.  b) On November 9, 2022, at the above addressed jobsite, employees performing sheeting of a residential roof were exposed to fall hazards of approximately 26 feet.  c) On November 9, 2022, at the above addressed jobsite, employees obtaining materials and accessing a ladder from an exposed second story landing were exposed to fall hazards of approximately 10 feet.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) On or about November 9, 2022, at the above addressed jobsite, employees were using an extension ladder which did not extend three (3) feet above the upper landing being accessed.   No abatement certification or documentation is required for this item.
